Evolve Personal Performance – Youth Participant Waiver & Release of Liability

(To be acknowledged by Parent or Legal Guardian)

By checking the acknowledgment box on the previous page upon sign up for class, I confirm that I am the parent or legal guardian of the minor participant (“Youth Participant”) and that I am voluntarily allowing them to participate in fitness-related activities offered by Evolve Personal Performance.

Program Participation & Supervision

I understand that the Youth Participant may take part in fitness classes, personal training sessions, or other structured programs led by a certified personal trainer contracted through Evolve Personal Performance. I acknowledge that youth participation is only permitted when supervised by a trainer and/or parent or guardian, and that unsupervised Open Gym access is not permitted for minors unless otherwise explicitly approved in writing.

Assumption of Risk

I understand that physical activity involves inherent risks, including but not limited to muscle strains, sprains, falls, equipment-related injuries, and other physical injuries. I acknowledge that these risks exist even when activities are supervised and conducted with appropriate care and instruction.

I accept full responsibility for evaluating whether the Youth Participant is physically and medically able to participate. I understand that it is my responsibility to disclose any medical conditions, injuries, limitations, or concerns related to the Youth Participant prior to participation.

Health & Medical Acknowledgment

I certify that the Youth Participant is in good health and physically capable of participating in fitness activities, or that I have chosen to allow participation despite known conditions. I understand that I may seek medical clearance prior to participation and that participation is voluntary and at my own discretion.

I acknowledge that the Youth Participant has been instructed to stop activity and notify a trainer or supervising adult if they experience pain, discomfort, dizziness, or any concerning symptoms.

Rules & Responsibilities

I agree that the Youth Participant will:

• Follow all gym rules, instructions, and safety guidelines

• Use equipment only as instructed and appropriate for their age and ability

• Be respectful of staff, equipment, and other participants

I understand that failure to follow safety guidelines may result in removal from participation.

Liability Release

In consideration of allowing the Youth Participant to participate, I hereby release, waive, discharge, and hold harmless Evolve Personal Performance, its owners, trainers, contractors, employees, and affiliates from any and all claims, liabilities, damages, losses, or expenses arising from the Youth Participant’s participation—whether due to negligence or otherwise—to the fullest extent permitted by law.

By checking the acknowledgment box on the previous page, I confirm that:

• I am the parent or legal guardian of the Youth Participant

• I have read and understand this waiver in its entirety

• I voluntarily agree to its terms on behalf of the Youth Participant
